Description

The job of Case Manager, Project ELECT was established for the purpose/s of optimizing student functioning through knowledge of problem solving, increased capacity, and effective linkages to resources for additional support.

 

Essential Functions include, but are not limited to:

 

  • Assist in the classroom, activities, and on field trips for the purpose of supporting student/clients   
  • Compile maintain, and submit (e.g. required student data) for the purpose of monitoring the program and student progress. 
  • Conduct group and individual support sessions and home visits for the purpose of monitoring plan implementation. 
  • Develop and implement Individual Plans for the purpose of ensuring that student/client is receiving proper support. 
  • Develop appropriate community networking and referrals within community- based organizations for the purpose of improving and expanding program services. 
  • Oversee student identification, outreach, recruitment, and intake for the purpose of ensuring that program utilization meets the needs and goals of the DCIU.
  • Provide ongoing information and assistance to the program coordinator and colleagues for the purpose of ensuring program success. 
  • Represent Project ELECT and the DCIU at assigned meetings and events for the purpose of communicating goals and objectives of the program. 
  • Maintain up-to-date notes, student logs and records for the purpose of maintaining required student and program data. 
  • Perform other related duties as assigned for the purpose of ensuring the efficient and effective functioning of the work unit. 
  • Support the DCIU mission and goals set by the Executive Director and the DCIU Board of Education and the Special Programs Director (e.g. Adhere to DCIU Board policies and procedures) for the purpose of maintaining and enhancing DCIU services.

 

Minimum requirements:

 

  • Community College and/or Vocational School degree with study in job related area
  • Bachelor's degree preferred
  • Job related experience is desired
  • Knowledge of community resources
  • Ability to keep records/reports
  • Good verbal and written communication skills
  • Ability to work on a computer
  • Valid driver's license
  • Reliable means of transportation

This is a 10-Month Position (188 days)
